The following forms are needed in order to begin processing your request to allow PHLEX Interstate Tax Services handle most of your permit, registration and licensing paperwork, along with tax reporting services.  

You will need Adobe Acrobat Reader installed on your computer in order to view and print these forms.  If you click on one of the form links below and you receive an error, you will need to click here to download the latest version of Acrobat Reader Free of Charge to Continue.

Acrobat Reader is free of charge.  It takes about 20 minutes to download and install on your computer.

Click on each link one at a time.  Please Note: Depending on how fast your Internet Connection is, the form can take up to a minute to load.  

You can fill out each of our forms online using your computer.  After you're finished with each form, be sure to print it out by clicking either on "File, then Print" or click the printer icon within Acrobat Reader.  After the form is done printing, close that form and come back to this page and click on the next form and do the same.
